Established in 2003, FSQ Pharmacy Holdings, LLC, a subsidiary of Five Star
Quality Care, currently operates
six institutional
pharmacies and
one mail-order
location. Our institutional service area includes the states of
Nebraska (three locations), Iowa, Missouri, Wisconsin, California (Los Angeles area), North Carolina, South Carolina, Georgia, Virginia, Maryland, and Delaware. Our pharmacy
division fills prescriptions for more than
8,800 institutional
clients in
10 states and
4,300 mail order
clients in
39 states on a
monthly basis.
